Cwm Taf Morgannwg have been working with our communities to develop our Dementia Connector. This role was coproduced by people with lived experience across the Cwm Taf Morgannwg region and is an integral part of our regional approach to the delivery of the All Wales Dementia Care Pathway of Standards.

It also directly links to the priorities of the Cwm Taf Morgannwg Regional Partnership Board which brings people together to improve services and support in Cwm Taf Morgannwg. Age Connects Morgannwg were successfully appointed to run the project and they are currently recruiting a Dementia Matters Programme Manager. The role will seek to support people who have been diagnosed with dementia regardless of age.
