VAMT is host employer to two Community Coordinators who are part of a Team which operates across Merthyr Tydfil and Rhondda Cynon Taf.
The Coordinators help individuals and their carers to access services and activities which will help them to maintain their independence and improve their health and wellbeing.
Anyone can refer to the team, and the type of support that they can source includes befriending; benefits checks; small home adaptations; shopping/cleaning/gardening as well as condition-specific support, such as dementia.
The team has strong connections with Local Authorities and is integrated within the Initial Support Services Team (Keir Hardie Health Park) and Single Point of Access Team in RCT (Ty Elai). This enables them to offer direct advice and information in relation to preventative services available within the local areas.
The Coordinators produce a monthly information bulletin promoting activities that are taking place in the month ahead. If you would like to receive a copy directly, or if you are a community group or organisation who would like to promote your services via the bulletin, then contact any of the Community Coordinators.
One Coordinator has a specific remit around Health, and in particular, brokering connections between third sector organisations and Primary Care settings to help raise awareness of the diversity of third sector provision. If you are an organisation who would like to connect with Primary Care professionals, then contact Deanne.
