Community Coordinators
VAMT is host employer to a Community Coordinator who is part of a Team which operates across Merthyr Tydfil and Rhondda Cynon Taf.
The Coordinators help individuals 50+ and their carers to access services and activities which will help them to maintain their independence and improve their health and wellbeing.
Anyone can refer to the team, and the type of support that they can source includes befriending; benefits checks; small home adaptations; shopping/cleaning/gardening as well as condition-specific support, such as dementia.
The team has strong connections with Local Authorities and is integrated within the Initial Support Services Team (Keir Hardie Health Park) and Single Point of Access Team in RCT (Ty Elai). This enables them to offer direct advice and information in relation to preventative services available within the local areas.
The Coordinators produce a monthly information bulletin promoting activities that are taking place in the month ahead.
