Community Coordinators
VAMT is host employer to a team of Community Coordinators who operate across Merthyr Tydfil
The Coordinators help individuals 50+ and their carers to access services and activities which will help them to maintain their independence and improve their health and wellbeing.
Anyone can refer to the team, and the type of support that they can source includes befriending; benefits checks; small home adaptations; shopping/cleaning/gardening as well as condition-specific support, such as dementia.
The team has strong connections with Local Authorities and the Initial Support Services Team (Keir Hardie Health Park)
