We can connect people to community activities, groups and services to help support their social, practical and emotional needs. This could include working with other third sector organisation to access:\n  <\/p>\n\n  <ul>\n    <li><b style=\"font-weight:700;\">Befriending<\/b> \u2013 combatting loneliness through phone or in-person companionship<\/li>\n    <li><b style=\"font-weight:700;\">Benefits checks<\/b> \u2013 making sure people receive what they\u2019re entitled to<\/li>\n    <li><b style=\"font-weight:700;\">Home adaptations<\/b> \u2013 improving safety and accessibility<\/li>\n    <li><b style=\"font-weight:700;\">Practical support<\/b> \u2013 help with shopping, cleaning, and gardening<\/li>\n    <li><b style=\"font-weight:700;\">Condition-specific support<\/b> \u2013 such as dementia guidance and signposting<\/li>\n  <\/ul>\n\n  <h3><b style=\"font-weight:700;\">Strong community links<\/b><\/h3>\n  <p>\n    Our team works closely with Third Sector Organisations, Local Authorities, and Health to provide seamless support tailored to individual needs.\n  <\/p>\n\n  <h3><b style=\"font-weight:700;\">Referrals are welcome<\/b><\/h3>\n  <p>\n    Anyone can refer to our Community Coordinators.
